How to write a PDE from spherical to cylindrical coordinates, 1D axisymmetric

I need to write an equation like this:

this equation it's written in spherical coordinates, I am using 1D axisymmetric so COMSOL use the cylindrical coordinates as coordinate system. I am using Coefficient Form Pde, so the form of equation is:

if now I set c=D I'll get:

how can I add the "r^2" terms? Thanks if someone will answare me !!
